Article Abstract:
A diagnosis of death based on the irreversible loss of brain-stem function has both advantages and disadvantages compared with the more traditional diagnosis based on the permanent end of heart and lung function. Advocates of brain-stem death criteria argue that they enable death to be determined at the most scientifically appropriate moment and prevent the distress caused to relatives and carers by the prolongation of unnecessary treatment. However relatives may be suspicious of a diagnosis of brain-stem death since the patient's heart may continue beating after the ventilator has been disconnected.

An alternative to seclusion
Article Abstract:
A medium secure unit has introduced an intensive care area as an alternative to seclusion for disturbed patients. The facility is used for those exhibiting threatening behaviour. Secure living space allows constant care away from other patients in a pleasant environment. Confinement is not a punishment. Reintegration into the main unit is the target. Nurses encourage patients to understand the therapeutic value of the unit.

Medical health professionals are becoming increasingly anxious about the consequence of errors in clinical judgement and there are increasing requests for forensic opinion to support the care of difficult patients. Forensic community mental health nurses (FCMHN) have seen their role become increasingly autonomous, and they often take a prominent position in clinical decision making.
