The importance of good record-keeping
Article Abstract:
Community psychiatric nurses in Northumberland, England, undertook a project to improve patient record-keeping in their service. They had noted a wide disparity in record keeping within their service, and felt that a standardised system would promote the nurses' confidence in their own ability to maintain effective records. A working group produced a new, more patient-centred, format for records which provides more information, identifies actions for health care interventions, and improves communication between health workers.

Thinking reflectively: solutions through action learning
Article Abstract:
Jane Pennington, clinical nurse specialist, Stuart MacKay, radiographer lecturer, and Joy, a trust board non-executive director, all benefited from action learning. Pennington took advantage of action learning to discuss her ideas on collaboration and teamwork. MacKay found that action learning helped him to deal with problems more effectively and become more self-aware. Joy found action learning helpful in personal development.
