The right to say no
Article Abstract:
A 1998 UK court ruling that women had the right to refuse recommended medical treatment, even if doing so could harm their unborn child, clarifies the legal position but presents challenges for midwives. Midwives have traditionally supported the rights of women to make choices in childbirth, but dealing with a choice which potentially threatens the life of the mother or child is not easy. The midwife must continue to undertake her professional duty to support and represent the mother, but she should also seek support for herself from her superiors.

The maternity day unit (MDU) provides a safe cost-effective alternative to hospital admission for antenatal care in high-risk situations. The Whittington MDU, London, England offers individual and continuing care in a relaxed environment. Criterior for care include hypertensive problems, reduced fetal movement, intrauterine growth retardation and overdue pregnancies. Estimates suggest a unit with 4,000 deliveries per annum may save 456,000 pounds sterling per annum.
