The incident that occurred at Duke University Health System, Washington regarding the mismatch between patient blood type and donated organs causing the death of a teenager is reported. This issue has led the University to take its own measures of investigation for ensuring that all parties have accurate information about the blood type of the recipient and the donated organs.
