Article Abstract:
A clinical nurse specialist in respiratory medicine talks about the protocol he developed for writing and dispensing drugs to patients. He re-wrote the protocol six times until it met with the approval of various consultants, nurses, pharmacists and the drugs and therapeutic committee. The health authority lawyers also had to agree that it was correctly worded. The nurse is now dismayed to learn that working under his protocol is not officially recognised and he must do as doctors direct him to do. He feels that nurses have more to offer than this and their skills are being wasted.

Put to the test
Article Abstract:
The use of peak-flow meters or spirometry can play a valuable role in the management of asthma and chronic obstructive pulmonary disease. Peak-flow meters offer an inexpensive and quite straightforward way of monitoring elements of asthma care, including seasonal alterations in asthma patients affected by pollens or changes in climate. Peak-flow monitoring may also boost compliance with treatment. Spirometry is now developing a key place in respiratory medicine, with all except the basic hand-held devices giving several parameters.
