Nurse Pat Cooksley is considering taking action under the Public Interest Disclosure Act 1998 when it comes into force in 1999. Cooksley has attracted media interest since speaking out on the practices of her employer, Plymouth Hospitals NHS Trust, which she considered to pose a threat to patient care. The Act will offer protection to employees who speak out on issues including failure to meet legal obligations, criminal behaviour, and matters of health and safety. Legal experts believe the Act is a powerful tool for change and Cooksley hopes it will encourage more people to speak out.

Community psychiatric nursing is an exciting and rewarding profession, although it can be stressful. Community psychiatric nurses may be based at hospitals or in primary care teams. Their workloads may be large and they will see patients with a range of conditions including schizophrenia and eating disorders. A report, compiled by the Sainsbury Centre for Mental Health in 1997, lists the core competencies of a co-ordinated community mental health service. These include assessment of risk, therapeutic interventions, emergency response and ongoing support and monitoring.

The UK government established the 24-hour medical helpline, NHS Direct with a grant of 50 million pounds sterling, with the expectation of millions of pounds sterling savings made in doctor and hospital costs. The helpline is designed to provide a 24-hour medical service for users who have queries regarding medical matters. Nurses running the helpline claim the service would be improved if they were given more time to study and undertake research. The average salary for a telephone nurse is 18,000 pounds sterling per year, including share options.
