Tunnel crash inquiry told of signal faults and rescue delays
Article Abstract:
A collision in the Severn Tunnel in Dec 1991 led to 180 injured passengers and partial blinding of one of the train drivers. There was delay in reaching the injured as rescue teams were led to the wrong end of the tunnel. The inquiry heard that only four pagers were working that day, and that emergency signals were operating in the tunnel. Extra staff were withdrawn from duty before the crash as they had no flags or detonators. Emergency lights in the tunnel were ignored because of a record for false alarms.

Channel tunnel builders fined 90,000 pounds sterling
Article Abstract:
The five companies building the UK to France channel tunnel have each been fined 18,000 pounds sterling after the judge at a two day hearing ruled there had been a serious failure in safety procedures. William Cartman, 33, died after his head was crushed between a moving beam and fixed machinery in 1990 and was the sixth man to be killed in the project. This is the sixth conviction of the five-member Translink Joint Venture for breach of safety regulations.

Ombudsman's report criticises HSE over pesticide poison scare

There is concern about the possible health risks to humans and animals being exposed to chemicals used to preserve wood. About 150,000 homes in the UK are treated annually with chemical 'cocktails' to treat problems like dry rot. One individual took her complaint about the chemicals to the Health and Safety Executive (HSE.) They were criticised for not taking action against the firm who treated the timber.
