Creaming, skimping and dumping: provider competition on the intensive and extensive margins
Article Abstract:
Creaming or overprovision of services to low severity patients happens when there is a cost-based reimbursement. Cost-based reimbursement promotes competition among health care providers in order to attract patients even if the advantages do not surpass the costs. The existence of dumping, which is blatant avoidance of high severity patients, may also signify the presence of skimping. Skimping refers to underprovision of services to high severity patients.

A simple economic model, identifying relationship between health and self assessed health status determined by lifestyle, preferences, budget and time constraints and unobservable characteristics, is presented. A formulation empirical health production function is developed.
