Article Abstract:
Physician hospital organizations should focus on how generalists and specialists can work collaboratively to deliver high quality, cost-effective care rather than merely on reducing costs. Research suggests that the current pressure to reduce the number of specialists and to have generalists treat more complicated problems and act as gatekeepers to specialist care may be based on unfounded beliefs that this will reduce costs without compromising quality. Not only is more research needed in determining who offers superior care under what circumstances, organizations need to know how to build integrated, collaborative health care delivery systems.

Article Abstract:
Internists may be the best physician specialty to treat older adults with complex chronic diseases. Internal medicine specializes in diseases of particular organs, such as heart diseases, lung diseases, and gastrointestinal diseases. Many internists subspecialize to become cardiologists and gastroenterologists, for example. As people age, they tend to develop chronic organ diseases, and managed care organizations need to accept the fact that internists are more qualified to manage these patients than family physicians.
