Home help: A rapid response nursing team providing acute care in patient's homes has reduced hospital admissions
Article Abstract:
North West Anglia Health Care NHS Trust proposed to use cash from the health authority's 1997 winter pressures money to form a rapid response team (RRT) to provide 24-hour services for acutely ill patients in their own homes. The activities of daily living model is used to assess each patient following referral, and an individualized care programme is implemented. This avoids the need for admission to hospital. An evaluation of the service found a high quality of care with quicker recovery, patients felt they had more information than in hospital and more independence.

Nurses in Oxford, England, train some patients with chronic demyelinating neuropathies to administer their own doses of intravenous immunoglobulin (IVIg) in the home. Selection of patients to perform this function is based on strict criteria including age, motivation and accessibility of veins. Patients must also have a companion willing to assist, if required, and to stay near the patient during IVIg treatment. Patients must be stabilized on IVIg treatment before commencing the home programme, and they and their companion must attend a two-day training session.
