Improving safety for patients receiving a blood transfusion
Article Abstract:
The integrated care pathway was revised to improve and maintain patient safety during blood transfusions and to improve the documentation of blood transfusion episodes. It is very critical to make sure that the documentation is accurate and care is evidence based.

An evaluation on 'copying letters to patients' scheme is carried out in a primary care mental health team to find their experiences of receiving reports. Many patients show positive response towards trust in services and commitment to treatment.
