Improving team meetings to support discharge planning
Article Abstract:
A practice development project was established at Queen Elizabeth Hospital NHS trust, London, to improve multidisciplinary team (MDT) meetings and support effective hospital discharge. The objective of the MDT meeting was to discuss all patients needing multidisciplinary input and focus on patients, setting an action plan for their discharge, identifying the roles and responsibilities of the nurses, doctors, care manager, occupational therapist, physiotherapist, and discharge coordinators.

Norwich's Julian hospital has offered an innovative outreach resettlement scheme, run by ward nurses, to ease the transition for patients from hospital to home or the community, which is the most commonly faced problem. The success of the scheme reveals an emphasis on recruiting staff that would take on community-based roles.

Promoting nurse-led discharge must be a priority
Article Abstract:
The vital role of nurses in discharge should be encouraged in order to create cost-effectiveness as well as empower the nurses so that they do not feel afraid to take the responsibility of discharge due to the blame factor.
