Article Abstract:
A patient describes her unhappy experience of life at the Royal Infirmary of Edinburgh, Scotland. She spent two weeks as an inpatient on an old ward which is normally reserved for geriatric orthopaedic patients. She describes the shortage of time which the nurses, devoted though they are, have for patients, and she bemoans the unclean conditions in the ward and the lack of adequate equipment. She feels that the UK National Health Service has been poorly administered and financed for some years, and that anticipated improvements under the Labour government have failed to materialize.

Turning evidence into everyday practice
Article Abstract:
Health care projects, like Promoting Action for Clinical Effectiveness projects, help health care workers monitor effectiveness of interventions and demonstrate their benefits. They also boost staff morale. However, it is important that clinical quality evaluation becomes part of ongoing activity, rather than being related solely to the project. The support of senior management is important in this task, as are good relationships between management and clinicians, the attachment of importance to patients' experiences, and the establishment of new structures of organisation.
