Patient records improve with unified case notes
Article Abstract:
Qualified nurses are expected to maintain a certain level of record-keeping, but audits at two hospitals have found deficits and problems in nursing documentation. Nursing documentation was reviewed and a new system of multidisciplinary documentation within unified case notes (UCNs) were developed. UCNs were found to help with patient management and many nursing staff were more aware of the legal aspects of documentation. However some nurses noted that action plans were not always updated and some information was missing.

A model approach to care of a patient with alcohol problems
Article Abstract:
It is important that a patient with alcohol problems recognises that a problem exists. In many cases, such patients will need professional counselling, especially if they have long-standing emotional difficulties. The patient must be encouraged to face the problem, being set both short-term and long-term goals. Nurses must be prepared to accept patients with alcohol problems, even though this may be very difficult, and must be willing to listen, looking beyond merely physical needs.
