Article Abstract:
The Elmore Community Support Team takes care of people who are not served by current agencies in Oxford, England, and are termed 'difficult to place'. This group is made up of the chronic, multiple disabled. There are three stages of work carried out by the team, acting as an counsellor for both client and agency, working out a package of care for the client with the agencies, and carrying out the role of case manager for the client.

Article Abstract:
A short term care unit for adults with learning disabilities gives families the chance to take a break, and offers a telephone help services and 24 hour admissions. The unit is non-institutional and activities are directly related to the needs of each client. It has close links with several community learning disabilities teams
