Systems linkages: developing the ties
Article Abstract:
The effective delivery of care to meet the needs of the elderly rests on the collaboration of different service systems. The formation of an integrated system goes through different stages. The first stage is the Interest Group which is composed of members of one system having a common interest. The Task Force is the next stage wherein members work together in an effort to accomplish a definite objective. The third stage is the Council wherein the members are regarded as experts in their own specific fields. The last stage is the formation of a coalition where the linkage assumes a bureaucratic form.

The role of the social worker in interdisciplinary geriatric teams
Article Abstract:
The emergence of gerontological social workers as an important part of health care service can be attributed to the expanding aging population. Their roles focus on easing the physical, psychological and social well being of geriatric patients. Social workers belonging to interdisciplinary teams conduct biological, psychological and social assessments of the patient, determine the most appropriate care management program and serve as the link between patients, their families and the community.
