Rehabbing Medicare: is managed care a cure-all or just a crutch?
Article Abstract:
The Health Care Financing Administration (HCFA) reports 3.1 million Medicare patients enrolled in 264 managed care programs by Feb 1995. Older Americans usually save money by joining such programs as health maintenance organizations (HMOs), but they lose the freedom to choose their doctors and hospitals. The HCFA determines an adjusted average per capita cost for each county. Most HMOs retain savings because enrollees are healthier than average. Friendly Hills, La Habra, CA, uses nurse practitioners and studies possible drug interactions to prevent unnecessary emergencies.

Albuquerque, NM, hospitals have been using a form of integrated financing and health care delivery since as early as 1986 when Presbyterian Healthcare Services and St Joseph Healthcare System joined forces to create Health Plus, a 57,000-member health maintenance organization that is now fully owned by Presbyterian. Albuquerque's eight hospitals belong to one of four health care systems that each operate under an accountable health plan and an integrated network. Healthcare reform will probably lead to further consolidation of hospital services.

United HealthCare Corp. has acquired MetraHealth Companies Inc. for $2.3 billion, but most of the 10 million new members are covered by expensive indemnity policies and may not stay loyal to United. United may attempt to consolidate facilities in areas where both companies have HMOs, and some analysts are suggesting that the company may offer a hybrid indemnity-HMO plan for the new members. Analysts also believe that United should sell off HMOs that it cannot merge together and spin off some of the new indemnity business.
