Article Abstract:
A Belfast, UK, hospital established a project to solve the conflict between nurses' hands-on work and paperwork. The record keeping and report writing skills of nurses proved to be mainly bad, which resulted in training being provided. The hospital has introduced a care planning system which has kept patient care as the focal point, whilst improving documentation and communication and involving patients and relatives in their care plans.

Psychiatric nurses have too much paperwork. Some nurses feel that the main reason for keeping notes is to cover themselves in case of problems rather than to ensure that patients are given continuous care.
